Hospice RN Case Manager
Service Area: Saint Johns to Mt Pleasant (North Territory)
Schedule: Monday – Friday, 8:00 AM – 5:00 PM

Your Role & Impact
Responsibilities
As a Hospice RN Case Manager, your expertise will provide comfort and peace of mind to patients and families. Your responsibilities will include:
✔ Holistic Care: Conduct comprehensive assessments to create individualized care plans.
✔ Hands-On Support: Deliver direct nursing care with compassion and skill.
✔ Seamless Communication: Keep care teams, physicians, and families informed every step of the way.
✔ Empowering Loved Ones: Educate and support families as they navigate this journey.
✔ Proactive Monitoring: Identify and address changes in condition promptly to ensure optimal comfort.
✔ Excellence in Action: Follow care plans and physician directives with professionalism and heart.
What You Bring
✔ Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Bachelor’s).
✔ Active RN license in Michigan.
✔ At least one year of clinical nursing experience
✔ Hospice experience is Highly referred
✔ Physical Requirements: Ability to sit, stand, bend, lift, and move intermittently, with the capability to lift 50-100 lbs.
✔ Reliable transportation for patient visits.
✔ Strong organizational and multitasking skills, with a passion for compassionate care.
